APS Healthcare manages two programs for Wyoming EqualityCare (Medicaid) -- Healthy Together, the state's health management program, which recently helped the Wyoming Department of Health avoid more than $12.7 million in unnecessary healthcare costs, and utilization management and peer review services for providers of EqualityCare.

The Wyoming EqualityCare total population health management program, Healthy Together, is aimed at improving health outcomes and reducing costs for clients of EqualityCare patients with chronic illnesses, such as depression, asthma, diabetes and heart disease. APS helps these clients receive one-on-one support from a health coach, educational materials to encourage self management of their disease and assistance with coordinating their care among multiple healthcare providers. Healthy Together is a free benefit for all Wyoming EqualityCare clients.
