You may qualify for a Health Buddy
If you are an EqualityCare client, you may qualify to receive a Health Buddy. The Health Buddy can assist people diagnosed with heart and lung problems, as well as diabetes and depression.
The Health Buddy device plugs into the telephone line and will ask you questions about your health and how you are feeling. The information is sent to an APS nurse who will call and stay in touch with you, should you need help.

Utilization management (UM)
APS works with your doctor and hospital to make sure the healthcare services meet your needs.
Case management (CM)
APS nurses help you get the most from your healthcare benefits. The nurses help you understand your situation. They work with you, your family and your healthcare providers to improve your health.
Healthy Together program
While you may have a chronic illness, you don't have to be chronically ill. There are daily choices you can make to feel better. The following, services are offered to Medicaid enrolled recipients.
The Healthy Together program is free and voluntary. It provides information and guidance if you have:
Healthy Together does not replace the care of a doctor. The program provides
- Information on health conditions
- Guidance on how to feel better
- Help on how to follow doctor's orders
Medicaid enrolled recipients may be called or receive a letter from APS staff about these services. Healthy Together does not change the services you get now. Your healthcare benefits will not change.

Asthma
Asthma affects more than 20 million Americans. People with asthma have symptoms from coughing to severe shortness of breath and wheezing. APS can help people with asthma breathe their best.
